A neck pain evaluation combines your history, a physical and neurological exam, posture and movement analysis, and imaging when it is needed. The goal is to find what is driving your neck pain and build a plan that fits.
What Happens at the First Visit?
We start by asking when the pain began, what triggers it, whether it travels into the arm or head, and how it affects sleep and work. We also ask about injuries, desk setup, and your goals.
The exam checks neck range of motion, tenderness, and strength. A neurological screen tests reflexes, sensation, and grip and arm strength to see whether a nerve is involved. Dr. Paul Fisher holds advanced certification in Selective Functional Movement Assessment, which helps us see how the neck, upper back, and shoulders move together.
Posture and X-Ray Analysis
We look at head position, shoulder alignment, and upper back posture while sitting and standing. When X-rays are appropriate, they can show alignment, disc space narrowing, and arthritis. Dr. Fisher’s clinical training includes Department of Defense hospital rotations in radiology, which helps him interpret imaging alongside the exam.
When Is an MRI Needed?
An MRI is usually reserved for significant or worsening arm weakness, signs of spinal cord involvement, a history of trauma, or symptoms that do not improve with care. Most neck pain does not need an MRI at the first visit.

- What is causing my neck pain?

- What can I change at work and at home?
